| Initial Visit Thank you for choosing Red Hill Orthodontics for your orthodontic needs. We are dedicated to providing excellent orthodontic care in a warm and friendly environment. As a courtesy to your dentist, your initial visit is complimentary. At Red Hill Orthodontics you will be greeted by one of our experienced Treatment Coordinators. They will review your completed Medical History Form with you (this form is usually mailed to you prior to your first visit with us). At this visit, digital photographs are taken as well as any necessary radiographs (X-rays). If you have had a panoramic radiograph taken at your dentist’s office within the last 6 months, please let us know and we will obtain it from your family dentist. These photographs and X-rays help Dr. Rahimi and Dr. Eslambolchi diagnose your malocclusion and determine the best possible treatment option. You will then personally meet one of our doctors, who will perform a thorough orthodontic examination. Once the examination has been completed, the doctor will explain your diagnosis and treatment plan to you, as well as the benefits of orthodontic treatment. One of our treatment coordinators will demonstrate what can be accomplished regarding your particular case using a special computer program. Once all your questions have been answered, we will then provide you with a detailed plan that outlines the recommended treatment goals and estimated fees. If you have dental insurance, a predetermination form will be provided so that you may determine what coverage you have. There is no fee for this initial consultation and it is about 60 minutes in duration. An appointment will be scheduled for you to start your treatment if you choose to do so before leaving our office. Please help us be prepared for your first appointment by completing this Patient Information and Medical History Form. Download and print the form, fill it in, and return it to our office. >> Adult Form: Word Format | PDF Format >> Child Form: Word Format | PDF Format If you're unable to read PDF files, you can download Acrobat Reader free from Adobe. Regular Appointments The appointment when you get your appliances usually takes from 45 to 60 minutes. You will then see us at regular intervals 4 to 8 weeks apart for an adjustment, which takes anywhere from 15 to 30 minutes. Patients are usually seen by appointment only. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. If you cannot keep an appointment, kindly give us 24 hours notice if possible. Cancel only if it is an emergency. It is difficult to reschedule you since most appointments are made 4 to 8 weeks in advance. Cost of Treatment The cost of orthodontic treatment is variable and depends largely on the severity of the patient's problem and the amount of time it takes to correct the problem. You will be able to discuss fees and payment options before treatment begins. Our office offers payment plans to suit your budget. In an effort to keep orthodontic fees down while maintaining the highest level of professional care, we have established this financial policy:
Insurance Prior to your initial visit with us please check with your insurance company to see how much coverage you may have for orthodontics. If you have orthodontic insurance coverage, we will be happy to assist you to determine the coverage you have available by providing you with a predetermination form to send to your insurance company. Orthodontic treatment is provided to the patient, and not to an insurance company. Therefore, patients are responsible for their payments to our office and we do not bill your insurance company directly. This is true of all orthodontic offices in Canada. We will try our very best to help in every way we can to answer any of your insurance questions. |
